The consequences of renal failure may be remembered using the mnemonic “MAD HUNGER”:

  • Metabolic Acidosis

  • Dyslipidemia (especially increased triglycerides)

  • Hyperkalemia

  • Uremia

  • Na+/H2O retention

  • Growth retardation and developmental delay

  • Erythropoietin failure (anemia)

  • Renal osteodystrophy

  • Definition:

    • Generally defined as abnormality in kidney structure or function generally resulting in eGFR of ≤60
      • Having a kidney transplant means abnromality in kidney structure automatically classifying at CKD
  • Classification

    • Cause, G1-G5 based on eGFR, Albuminaemia (A1-3) (CGA) classification has now surpassed the previous staging

Management

  • First line is lifestyle management
    • Screen for sleep apnoea in overweight/obese patients
  • 4+3 approach
    • RAS-inhibitors
    • SGTL2-i
    • BP < 120
    • Statin
