    • ECG - ischaemia, arrhythmias UA and urine culture
  • Bloods
    • Blood cultures x2
    • FBC - anaemia, thrombocytopaenia
    • DIC panel (e.g. INR, aPTT, fibrinogen)
    • EUC - AKI
    • ABG - quantify P/F ratio, hypercapnoea, pH status and lactate
    • LFTs - ischaemic hepatitis, hyperbilirubinaemia
    • Troponin
  • Imaging
    • Chest X-ray - assess respiratory source and pulmonary oedema
    • Echocardiogram - sepsis-induced cardiomyopathy
    • Abdominal imaging

Management

  • ABCs
  • Restore haemodynamics
    • IV fluids boluses 30mL/kg in the first 3 hours
      • Signs of adequate resuscitation
        • MAP ≥ 65mmHg
        • Urine output ≥ 0.5mL/kg/hr
        • Improving lactate
    • Pressors if fluids insufficient
  • Treat the underlying infection with broad spectrum antibiotics
  • Source control
  • Potentially vitamin C, thiamine and hydrocortisone
