- SOFA Score can be used for sepsis severity score
- Antibiotics
- Tazoscin
- Vancomycin
- ±Azithromycin if suspicious of respiratory cause
Investigations
- Bedside
- ECG - ischaemia, arrhythmias
UA and urine culture
- Bloods
- Blood cultures x2
- FBC - anaemia, thrombocytopaenia
- DIC panel (e.g. INR, aPTT, fibrinogen)
- EUC - AKI
- ABG - quantify P/F ratio, hypercapnoea, pH status and lactate
- LFTs - ischaemic hepatitis, hyperbilirubinaemia
- Troponin
- Imaging
- Chest X-ray - assess respiratory source and pulmonary oedema
- Echocardiogram - sepsis-induced cardiomyopathy
- Abdominal imaging
Management
- ABCs
- Restore haemodynamics
- IV fluids boluses 30mL/kg in the first 3 hours
- Signs of adequate resuscitation
- MAP ≥ 65mmHg
- Urine output ≥ 0.5mL/kg/hr
- Improving lactate
- Pressors if fluids insufficient
- Treat the underlying infection with broad spectrum antibiotics
- Source control
- Potentially vitamin C, thiamine and hydrocortisone
